What it’s about:

In this role, you will have the opportunity to work as part of a team with adults that have been identified as needing intensive interventions related to their mental health diagnosis. Individuals will have assessments done every four months to pinpoint their on-going needs, strengths being developed, and the level of interventions needed. Interventions are done with a team approach in ACT services. The team includes at least a team supervisor, nurse, psychiatrist, and ACT CM. During the assessments, the individual’s road map to recovery (also known as a person-centered recovery plan) is co-created by the ACT CM and individual.

The ACT CM will have the opportunity to not only handle the assessments, but also carry a caseload and provide direct interventions utilizing evidence-based practice curriculums to assist those with their mental health related challenges. You will provide most of the interventions for your assigned caseload in their homes or out in the community. The ACT CM will have individuals on their caseload that need assistance getting to their clinic appointments and the ACT CM will provide transportation when other resources have been exhausted. We do have performance measures that we want you to achieve each month.

These areas are important because they help us see that the individuals you’re responsible for are receiving services to their needed level of intervention. As this is one of our most intensive levels of care, we prefer candidates to have experience working with adults with severe and persistent mental illness. We do provide training and shadowing to help equip you to do the job successfully.

What we’re looking for:

An individual who has a passion for helping others that is also organized and able to meet deadlines consistently. An abundance of both patience and persistence is needed in this role. What your college may not have told you about social service careers is that there is always paperwork involved! Documentation is essential to showing that engagements and services were provided in a timely and consistent manner.

We use an electronic health record to capture our documentation. Therefore, we need someone that has excellent computer operation skills. As ACT is a team approach, we need a team player who asks for help, communicates well, and will accept constructive feedback. The types of interventions provided

every day can vary greatly and we need someone that is able to be flexible in dealing with the challenges and changes that occur.

Thinking about it?

This position will require communication and coordination with other community agencies. Having someone that is friendly and professional will go a long way to building successful relationships with these other agencies. As an ACT CM, you’ll be traveling around the county where you’re working. Therefore, you must have reliable transportation and a willingness to drive. We do currently reimburse for mileage incurred for work-related travel.

What you must have:

You must have a bachelor’s degree in one of the fields listed at the top; your unofficial transcripts are required initially to verify the degree, with an official copy required later if you’re hired on.

We require QMHP-CS credentialing for this position, and your degree allows us to do that. Due to the type of agency that we are and that you will be driving to perform your work, a good driving record for the past three years, valid Texas driver’s license, and proof of auto insurance is required.
