then the Youth Wraparound Facilitator may be the position you’ve been looking for!
What it’s about:
In this role, you will have the opportunity to apply your understanding of children’s behavioral health and your cultural sensitivity to work collaboratively with families, their supports, other professionals, and community partners to develop individualized, family determined, strength-based plans of care. Wraparound is an evidence-based planning process used to support youth at risk of being placed out of their home and communities due to the significance of their behaviors. You will be responsible for coordinating their care by facilitating team meetings once a month, and meeting with families weekly to monitor their progress. Wraparound Facilitators have a unique opportunity from other case management positions because they work with 10 or less families, and work with them for longer periods of time. Consequently, you will develop meaningful relationships with the individuals you work with. We provide reimbursement for work related travel, and you will receive a company laptop and cell phone to perform your duties. There are opportunities for internship and supervision hours for those in graduate or post graduate programs preparing for licensure. Pecan Valley Centers is also an agency that qualifies for Public Service Loan Forgiveness (PSLF).
What we’re looking for:
A flexible, organized, creative individual who is passionate about helping families and possesses strong communication and computer operation skills. A successful facilitator will recognize the planning process in Wraparound is individualized and will be unique to each family. Flexibility and creativity are important because an effective facilitator will modify their approach based on the preferences of the family and the unique demands of each situation. Being organized is also desired to ensure timely documentation, effective time management, and ability to work independently. Excellent communication skills, including active listening and nonverbal, are important for facilitators in communicating between all team members and facilitating team meetings. We use an electronic health record for documentation and other tasks that will require comfortability in using Microsoft Word, Teams, Outlook, Excel, and PowerPoint. Ability to browse the web for resources for our families is also needed.
What you must have:
You must have a bachelor’s degree in one of the fields listed at the top; your unofficial transcripts are required initially to verify the degree, with an official copy required later if you’re hired on. We require QMHP-CS credentialing for this position, and your degree allows us to do that. Due to the type of agency that we are and that you will be driving to perform your work, a good driving record for the past three years, valid Texas driver’s license, and proof of auto insurance is required. You’ll also need to be able to sit for an extended period of time, reach overhead, and safely walk over various types of terrain.
